This was actually a recognition of her status.
In fact, the head nurse does hold the highest position in the operating room. It's not uncommon to see the head nurse scolding whoever she displeases, even the surgeon isn't exempt.
However, just like a circulating nurse may reprimand an intern and even replace them into a first assistant role, this doesn't necessarily mean the circulating nurse is more esteemed than the intern.
Nurses and doctors are entirely different systems.
It's somewhat like a husband fearing his wife; this fear might not really be fear, but a form of love, a gentleman's conduct, a sign of tolerance and magnanimity.
