Even if they did not ask him or he denied there being anything affecting him to cause such a change, rumours could spread from those who held those suspicions enough to talk about them with others.
Even if they chose to keep quiet about it, some people would want to confirm if what they believed was true, especially if they had some motive to do so.
A motive for doing so could often be quite simple.
They could just be curious enough to know the truth.
