Chapter 367 - Chapter 367: A Kid At The Controls, A Deadly Reputation, And 5418M!

"Hello there viewers...Keiko here again...*Keiko proceeds to say this while traditionally bowing to the viewers as usual. To which Keiko then proceeds to bring her attention and eyesight back up again, in order to continue to address the viewers. Keiko also now proceeds to briefly clear my throat, as Keiko then proceeds to continue, to properly address the viewers*...

Now then viewers, for this chapter I will once again be covering several more transportation related accidents in history...And for this particular chapter viewers as like the last one, we will be covering another three of them...

And each of them, as shown by the chapter title of this particular chapter, is going to be covering one aviation accident, that was attributed to a unique case of 'Pilot Error'...

Another, will be attributed to a 'Runway Overrun Caused By Pilot Error'...

And the final one viewers, which is another rail accident that was attributed to a case of 'Toxic Work Culture' so too speak...

...Now then viewers, lets begin with the first of the two aviation accidents...

And for this one, we will start by going all the way back, to March 23, 1994...

And on this particular day back in...Aeroflot Flight 593...Which was a flight operated by the Russian Airline 'Aeroflot,' between Sheremetyevo International Airport, in Moscow, Russia over to Kai Tak Airport, in what was a the time British ruled Hong Kong. And at the time of the crash. The route in question at the time, was being operated by an Airbus A310-304...

On board Flight 593, were three pilots, nine flight attendants, and sixty-three passengers...

The acting non relief captain of flight 593, was Andrey Viktorovich Danilov, aged 40, who was hired by Aeroflot in November 1992. He had accrued over 9,500 hours of flight time, including 950 hours in the A310, of which 895 hours were as captain...

The First Officer of flight 593, was one Igor Vladimirovich Piskaryov, aged 33. Who was hired by Aeroflot in October 1993. He had 5,885 hours of flight time, including 440 hours in the A310...

And finally the relief captain of flight 593, or the crew member who would become the main factor of this particular air accident, was one Yaroslav Vladimirovich Kudrinsky, aged 39. Who was hired by Aeroflot in November 1992. He had over 8,940 flying hours, including 907 hours in the A310. Kudrinsky also had experience in the Yakovlev Yak-40, Antonov An-12 and Ilyushin Il-76...

And as for what wound up happening? Well while flight 593 was at cruise, relief captain Kudrinsky, had decided to allow his two children into the cockpit. As on this particular flight, Kudrinsky had brought his children with him on the flight to Hong Kong...

And tragically, one of his two children, who by the way he had allowed to sit in the captains seat. Without any kind of warning to alert anyone in the cockpit, had wound up inadvertently, and partially disconnecting flight 593s autopilot. And as a result of this, 593s ailerons, which was the portion of the autopilot that had disconnected. Wound up causing the aircraft to bank quite steep. And unfortunately, the crew was not able to react in time, and the first officer wound up pulling back on his control column to much, thus causing flight 593 to go into a corkscrew dive and also stall...

And unfortunately, by the time the crew had managed to bring the aircraft back to somewhat wings level, they had run out of altitude, and also out of time. As flight 593 wound up impacting a mountain, and the impact would wind up killing all seventy-five people on board instantly...

And now viewers, we will be moving onto the second of the two aviation accidents of this chapter...And for this one, we will be going all the way forward, to July 17, 2008...

And on this particular date TAM Airlines Flight 3054, which at the time was being operated by an Airbus A320-233... 

And was operating from Salgado Filho International Airport, in Porto Alegre, Brazil, over to São Paulo–Congonhas Airport, in São Paulo, Brazil...

On this particular flight, were two pilots, and four flight attendants. And on board with the six crew, were 181 passengers...

The captain was captain Henrique Stefanini Di Sacco, age 53, and captain Kleyber Aguiar Lima age 54. Yes viewers, both pilots on this particular flight were captains. And both pilots had been flying for over 30 years. Stefanini had logged 13,654 flight hours in his career, including 2,236 hours on the Airbus A320. And Lima had logged 14,760 hours, with 237 of them on the Airbus A320...

And for a bit of context with regard to the aircraft that was involved in this particular accident. A few days before the crash, the Airbus had to have one of its two thrust reversers shut down, due to it needing repairs. But the Airbus, was still cleared to fly with one of its thrust reversers inoperative...

But sadly, Flight 3054 would wound up overshooting runway 35L at Congonhas. With the aircraft crashing into a nearby TAM Express warehouse, as well as a nearby petrol station. The impact killing all 187 passengers and crew on board TAM flight 3054, as well as another 12 people on the ground. And as for the cause of the crash of TAM flight 3054? Well as the aircraft had only one operative thrust reverser, this had caused the captain to have to use a procedure for landing with only one thrust reverser functioning. But here's the thing viewers, at the time of this particular accident, there were two separate procedures for landing with only one functioning thrust reverser...

And tragically in this case, the captain wound up mixing up both procedures. As he wound up throttling back only the engine that had the functioning thrust reverser, while leaving the other engines throttle at full power. Which in this case, wound up causing TAM flight 3054, to overshoot the runway...

And now viewers, we will be moving onto the third and final transportation accident of this chapter...And for this one, we will be going back, to April 25, 2005...

And on this particular date, Rapid Train 5418M was operating between Takarazuka Station, and Amagasaki, which was seven station stops down the track. And for those of you viewers who don't know of just how sufficient Japan's railways are. Then I do strongly suggest, that you go and watch season 11 episode 4 of Top Gear...

And on board Rapid Train 5418M, were roughly 700 passengers...

And tragically, after a series of mistakes having been made by the driver of 5418M leading up to the accident, 5418M would wind up derailing on a sharp curve, at a speed of 116 kilometers per hour. Or roughly 72 miles per hour, the resulting derailment causing the first four carriages of 5418M to tip over. And slam into an apartment building next the railway line. And for those who are wondering, the speed limit on the curve in question, was only 70 kilometers per hour, or roughly 44 miles per hour...

A full 28 miles an hour faster then the allowed speed limit...

And tragically, 562 were injured in the derailment, and 107 were killed, including the driver of 5418M...

But the thing is viewers, I don't blame the driver for the derailment...

And as for why you may think that I have come to this decision? Well then I do highly suggest that you go and look that up...

Because while there are some who would tell you, I on the other hand would rather choose to respect those who were injured or killed...

I mean after all viewers, you need to treat people with the utmost respect...

And though I have said this in many of these same chapters before...

I will once again reiterate, that I don't mean any sort of disrespect, to those whose lives were lost by either this tragedy, or those like it...

As the only reason why I am mentioning them in these fanfics, is to help preserve the legacy of those who were injured or killed in these tragedies...

And also, to make sure that those who lost people in these tragedies, have some sort of another way to remember their loved ones...
