The surgical room under the Hokage's tower was quiet.
Early morning light slipped through the high windows, casting a faint glow across the walls. Two medical beds were placed side by side in the center of the room. On one bed lay Itachi Uchiha.
On the other hand, Sasuke Uchiha. Both were unconscious, prepared hours ago for the transplant. Their faces looked calm, but chakra pulsed faintly from them. Like waves rising and falling. Not dangerous, but strong.
The air smelled like fresh herbs and sharp antiseptic. A few sealing scrolls were stuck to the walls, glowing faintly with light blue runes.
They kept the chakra in the room stable. Around the beds, small shelves and tables held metal tools, clean, cold, and sharp. Everything was ready.
Kakashi stood at Sasuke's side, wearing a white medical robe. Tsunade stood at Itachi's side, also dressed in a matching white coat. Her face was serious, but calm. The two had gone over the plan many times.
"No turning back now."Tsunade sighed and crossed her arms.
"Let's begin," Kakashi said as he looked down at Sasuke's calm face.
Tsunade took the lead. She raised her hands, and soft green chakra glowed from her palms.
"I'll go first," she said.
She leaned over Itachi and carefully moved her hands to his face. She was gentle. Her chakra wrapped around Itachi's closed eyes.
Slowly, the tools lifted, tiny silver tweezers and narrow scalpels. Kakashi watched her closely. She was calm. Her movements were slow, practiced, and sure.
Within minutes, Itachi's eyes were removed. They were placed carefully in a special chakra-preserving solution. The scroll seals around the room glowed brighter for a moment, sensing the chakra flare. Itachi's chakra pulsed again. Deep, strong, but not wild.
Kakashi nodded to himself. Now it was his turn.
He moved over to Sasuke and raised his hands. His chakra glowed softly, cool blue with a tinge of silver. Slower than Tsunade, but just as careful, Kakashi started the same process. He steadied Sasuke's head with one hand while working with the other.
As he removed the eyes, Sasuke's chakra jumped for a second. Hot and sharp, like a blade scraping steel.
"Their chakra is reacting."Tsunade frowned slightly.
Kakashi checked one of the sealing scrolls. "It's normal. They're Uchiha. Even unconscious, their chakra remembers pain."
Tsunade nodded but stayed quiet. She used her green medical chakra to treat Itachi's nerves, making sure no damage remained.
Kakashi did the same for Sasuke, carefully healing the nerves and applying a small stabilizing seal behind his right temple.
The seal was special. It was a small spiral with three lines going outward. It would keep the chakra from flaring too fast once the transplant finished.
They worked in silence for the next few minutes. The only sound was the soft hum of the chakra monitors and the rustle of gloves against cloth. The chakra of both brothers was strong. It filled the room slowly like a tide.
When both sets of eyes had been fully removed, cleaned, and stored in their solution, the second step began.
"Here we go."Tsunade picked up the container holding Sasuke's eyes.
She placed Sasuke's eyes into Itachi's sockets carefully, using chakra to help them settle. It wasn't like a normal surgery. The chakra from the new eyes had to accept and merge with the new host.
Tsunade had done eye transplants before, but not between two Mangekyō Sharingan users. This was new for her.
At the same time, Kakashi did the same for Sasuke, placing Itachi's eyes into his. The chakra flared again. Stronger this time. The sealing scrolls flickered, one of them buzzing loudly before calming down.
Tsunade steadied her hand. "Easy," she muttered. "Don't push too hard yet."
But Kakashi didn't panic. He knew what was happening. The eyes were adjusting. He felt the chakra twist for a moment, then settle down.
He quickly applied clean bandages over Sasuke's eyes. Tsunade did the same for Itachi.
Then… silence.
The chakra stopped flaring. The room grew silent.
Tsunade moved over to a chakra monitor.
"Stable," she said. "We did it."
Kakashi leaned back against the wall and let out a soft breath. He looked at the two brothers, now lying side by side with bandages over their eyes. They looked peaceful again.
"They just need to rest," he said. "A day or two, and they can remove the bandages."
Tsunade nodded. She sat down on the nearby chair and rested her hands on her knees. "We're lucky. Their chakra systems didn't reject it. I was worried."
"They're Uchiha. Their chakra is strong, and I was sure there would be no rejection."Kakashi gave a small smile.
"I hope this gives them a new chance."Tsunade chuckled softly, then looked at Itachi's bandaged face.
Kakashi looked too. He didn't speak for a moment.
He thought of the pain these two had gone through. Brothers born into an apparently cursed clan, each walking paths soaked in blood. And now… they were still alive. And stronger than before.
But maybe, this time, they were stronger for each other.
He stepped away quietly, letting them rest.
He looked once more at the sealing scrolls on the wall. The chakra had settled. The transfer was complete. And he had nothing to worry about anymore, as the full results will be seen when the two will wake up.
As Kakashi left, the room fell into silence.
